The following are the forms that all volunteers must review, sign and agree to follow. All adult volunteers must complete the first 2 forms below, the Waiver and Job Hazard Analysis.

Any minors must have a parent/guardian complete all 4 forms below. Signature must be of the adult.

 

Our preference is for these forms to be completed and brought to your first cleanup of the year. We will also have blank signature pages at cleanups.​
